ASCA Waiver Request Form 09/07 Waiver Request Form:
  • Must be completed in order to send paper claims to National Government Services, even when an ASCA exception is met.
  • Must be completed for each provider/supplier number in order to send paper claims.
  • Supporting documentation must be included for each provider/supplier number separately.
  • Form must be completed and mailed to:
         National Government Services
         Attention: ASCA

         Indianapolis, IN 46250
