Dear Supplier:
AdminaStar Federal’s (ASF) policy, through its Small Business
Supplier Diversity Program, is to increase the business opportunities
for small business, veteran-owned small business, service-disabled
veteran-owned small business, HUBZone small business, small disadvantaged
business, and women-owned small business concerns.
To ensure our compliance with applicable government reporting requirements
in accordance with the Federal Acquisition Regulation (FAR) Part
19, we are evaluating our suppliers’ socioeconomic status
accordingly. With this in mind, we are asking our current and potential
suppliers to complete the attached Vendor
Profile Questionnaire.

- Submission of this form does not constitute approval of your

- Please submit any certifications of your firm’s Small
and Disadvantaged status with your vendor profile response. Return
