Initial Enrollment Questionnaire (IEQ)

A questionnaire issued to all beneficiaries three months prior to eligibility to Medicare by the Centers for Medicare and Medicaid Services (CMS). The questionnaire is an attempt to determine if Medicare will be primary or secondary before the first Medicare claim is submitted.
